Qualifications

Position Overview

The Case Management Program Director is an intermediate management role responsible for implementing the organization’s mission, vision, and strategic priorities into actionable plans and measurable results. This role provides support for the AILP and BSLP Co-Op programs and other case management programs. This position bridges between the programs and the Senior Management Team.

The Case Management Program Director partners closely with the Case Management Program Manager and Housing & Entitlements Manager along with the Senior Management Team to achieve an excellent standard of client care and staff support.

Primary Duties and Responsibilities

  • Provide overall leadership and management of agency-supported cooperative housing programs and case management services.
  • Develop, implement, and monitor best practices, policies, and procedures that enhance client care, treatment outcomes, and independent living.
  • Ensure consistent implementation of the agency's clinical philosophy, program objectives, and service standards.
  • Recruit, supervise, mentor, train, and evaluate program staff while promoting professional development and high performance.
  • Monitor staff productivity and ensure case managers achieve required service units across all billable categories.
  • Ensure timely, accurate, and complete clinical documentation in accordance with agency, county, and regulatory standards.
  • Collect, analyze, and report client and program data to measure outcomes, improve service delivery, and support continuous quality improvement initiatives.
  • Maintain compliance with Medi-Cal, county mental health contracts, licensing requirements, and all applicable regulatory standards.
  • Build and maintain collaborative relationships with community landlords to support client housing stability and successful tenancy.
  • Develop, implement, and monitor key performance indicators (KPIs) and management reporting systems to evaluate program effectiveness and operational performance.
  • Promote culturally responsive, equitable, and trauma-informed services that meet the needs of diverse client populations.
  • Monitor occupancy levels and establish effective procedures to minimize vacancies and facilitate timely unit turnover.
  • Oversee facility operations by developing preventive maintenance procedures and coordinating emergency preparedness and disaster response plans.
  • Utilize evidence-based practices and outcome measures to evaluate program success and improve client outcomes.
  • Represent the agency by participating in internal meetings, community partnerships, committees, and interdisciplinary workgroups.
  • Participate in the agency's rotating on-call schedule and perform other duties as assigned.

Minimum Qualifications

  • Active licensure or registration in good standing with the California Board of Behavioral Sciences (BBS) as one of the following: Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Associate Clinical Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), Associate Professional Clinical Counselor (APCC), or licensed Psychologist (Psy.D.).
  • Minimum of three (3) years of progressively responsible experience in behavioral health program management, clinical supervision, and program administration within a community-based behavioral health setting.
  • Demonstrated experience leading multidisciplinary teams, including hiring, training, coaching, supervising, and evaluating staff while fostering a positive, collaborative, and accountable work environment.
  • Strong knowledge of behavioral health case management, psychosocial rehabilitation, supportive housing, and recovery-oriented systems of care.
  • Experience ensuring compliance with Medi-Cal documentation standards, county behavioral health contracts, and applicable state and federal regulations.
  • Demonstrated ability to monitor program performance, analyze client and operational data, develop key performance indicators, and implement quality improvement initiatives that support evidence-based outcomes.
  • Knowledge of trauma-informed care, evidence-based therapeutic practices, and social rehabilitation principles, with the ability to integrate these approaches into program operations and service delivery.
  • Demonstrated cultural humility and competency in providing services to diverse populations, including individuals experiencing serious mental illness, co-occurring substance use disorders, HIV/AIDS, and housing instability.
  • Proven ability to establish and maintain collaborative working relationships with staff, clients, landlords, community partners, referral sources, and other stakeholders.
  • Excellent written, verbal, interpersonal, and presentation skills, with the ability to communicate effectively across multidisciplinary teams.
  • Strong organizational, leadership, and critical thinking skills, with the ability to manage multiple priorities, meet deadlines, and maintain productivity standards in a fast-paced environment.
  • Experience developing and implementing policies, procedures, workflow processes, and operational practices that improve service quality, compliance, and program efficiency.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook, and PowerPoint); experience using electronic health records (EHR) and other clinical documentation systems is highly desirable.
  • Ability to work flexible hours as needed, including occasional evenings, weekends, holidays, overnight on-call coverage, and participation in a rotating on-call schedule.
  • Demonstrated commitment to providing exceptional customer service, compassionate client care, ethical practice, and a recovery-oriented, person-centered approach to service delivery.
