Job summary

PRC Baker Places in San Francisco seeks a Leland House Program Director to lead a transitional housing program, aligning services with the agency’s mission and strategic priorities. We support formerly homeless adults living with co‑occurring mental health and substance use disorders by providing a continuum of integrated direct care services. Position Overview

The Leland House Program Director is an intermediate management role responsible for implementing the organization’s mission, vision, and strategic priorities into actionable plans and measurable results. This role provides support to the Leland House Associate Program Director, and Housekeeper. This role is a bridge between the programs and the Senior Management Team.

The Leland House Program Director partners closely with program managers and the Senior Management Team to achieve an excellent standard of client care and staff support. The Leland House Program Director also partners closely with external stakeholders and departments to achieve excellent client care.

Primary Duties and Responsibilities
  • Provide overall leadership and management of agency-supported transitional housing program.
  • Develop, implement, and monitor best practices, policies, and procedures that enhance client care, specified outcomes, and independent living.
  • Ensure consistent implementation of the agency's philosophy, program objectives, and service standards.
  • Recruit, supervise, mentor, train, and evaluate program staff while promoting professional development and high performance.
  • Monitor outcomes, staff productivity and ensure case managers achieve required service units across all billable categories.
  • Ensure timely, accurate, and complete documentation in accordance with agency, county, and regulatory standards.
  • Collect, analyze, and report client and program data to measure outcomes, improve service delivery, and support continuous quality improvement initiatives.
  • Maintain compliance with Medi‑Cal, county mental health contracts, licensing requirements, and all applicable regulatory standards.
  • Build and maintain collaborative relationships with community partners, and landlords to support client housing stability and successful tenancy.
  • Develop, implement, and monitor key performance indicators (KPIs) and management reporting systems to evaluate program effectiveness and operational performance.
  • Promote culturally responsive, equitable, and trauma‑informed services that meet the needs of diverse client populations.
  • Monitor occupancy levels and establish effective procedures to minimize vacancies and facilitate timely unit turnover.
  • Oversee facility operations by developing preventive maintenance procedures, coordinating emergency preparedness and disaster response plans, and residential needs.
  • Utilize evidence‑based practices and outcome measures to evaluate program success and improve client outcomes.
  • Represent the agency by participating in internal meetings, community partnerships, committees, and interdisciplinary workgroups.
  • Participate in the agency's rotating on‑call schedule and perform other duties as assigned.
  • Provide individual behavioral health counseling and case management designed to support transitional clients with mental health and/or substance use disorder needs.
  • Provide group therapy and rehabilitation designed to support transitional and non‑transitional clients with mental health and/or substance use disorder needs.
  • Coordinates with other providers to support residents in accessing primary medical care, mental health care, substance use disorder care, and other necessary/needed services to address client’s needs– including PRC Baker Places’ wrap‑around services.
  • Provide out reach to external providers, HV service providers and others.
  • Conducts initial and updated psychosocial assessments and/or ASAM assessments.
  • Develops individualized treatment plans and adjusts as needed.
  • Documentation to meet agency standards and timeliness.
  • Provide culturally competent services using harm reduction and trauma informed principles.
  • Responds to mental health crisis to provide immediate access to services, de‑escalation, and follow‑up care.
  • Provides rotating on‑call duties that may include responding to a crisis or de‑escalation outside of business hours.
  • Maintain a productivity target based on contractual requirements.
  • Complete and maintain 51/50 certification; responding when necessary for assessment and follow‑up.
  • Promote agency mission, purpose, and values in relationships with internal and external providers.
  • Maintain licensure or license eligible status, complete required annual trainings, maintain ANSA and ASAM certification.
  • Maintain CPR and First Aid certifications.
  • Comply with annual TB testing requirements.
  • Additional duties as assigned.
Minimum Qualifications
  • Actively licensed or registered with the California Board of Behavioral Sciences (BBS) as one of the following: LCSW, LMFT, Psy.D., ASW, AMFT, LOCC, APCC
  • Minimum two (2)years’ clinical experience working with adults with a history of mental health, substance use disorder, HIV/AIDS, and/or homelessness
  • Minimum three (3) years of supervisory and/or management responsibilities of direct care staff.
  • Experience working in or managing a transitional housing setting serving individuals with mental health and/or co‑occurring needs preferred.
  • Knowledgeable about San Francisco community resources that serve our population preferred.
  • Strong practical and theoretical foundation in clinical MH/SUD evidence‑based best practices.
  • Must possess exceptional verbal and written communication skills.
  • Proficiency in Microsoft Office products required.
  • Willingness to work non‑traditional hours as the job requires– including nights, weekends, and holidays.
  • Must maintain First Aid and CPR Certifications.
  • Must be able to pass CDSS, FBI, and DOJ background.
  • Effective time management and ability to meet deadlines.
  • Exhibit exemplary customer service, compassion, and care in the execution of all duties and interactions.
