Job summary

Viability, Inc. is seeking a Case Management Nurse (RN) in Springfield, MA, with regular travel to Worcester. The role offers a Monday–Friday schedule, a starting wage of $36.06 per hour, and a $500 sign-on bonus.

You will conduct assessments, coordinate care, complete prior authorizations, and ensure quality services for Adult Foster Care members. This position emphasizes a healthier work-life balance, with no hands-on nursing care, limited supervision, and opportunities to impact the community

Qualifications

• Massachusetts RN license required.
• Minimum 2 years of human services experience preferred.
• Valid driver's license and reliable transportation required.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently and manage priorities effectively.
• Demonstrated reliability, professionalism, and respect for diverse populations.

Responsibilities

• Conduct clinical eligibility assessments for Adult Foster Care members.
• Attend required in-person meetings and home visits.
• Complete prior authorizations and required documentation.
• Ensure compliance with program standards and contractual requirements.
• Work independently while collaborating with an interdisciplinary team.
• Assist with program organization, expansion, and staff training as needed.
