$60.00/hr - $64.00/hr

Shifts: Monday–Friday, 9:00 AM – 5:00 PM

Job Summary:

The RN Care Manager will play a key role in coordinating patient care through case management, utilization review, and resource planning. This non-clinical role is ideal for RNs passionate about improving patient outcomes, streamlining care delivery, and collaborating with interdisciplinary teams in a structured healthcare setting.

Key Responsibilities:

  • Manage patient care plans and ensure effective resource utilization.
  • Collaborate with providers, payers, and care teams to optimize patient care outcomes.
  • Conduct assessments and implement care management strategies based on clinical guidelines.
  • Review cases for medical necessity and coordinate appropriate levels of care.
  • Utilize the EPIC system to document and track patient progress.
  • Support discharge planning and transitions of care.
  • Adhere to regulatory and compliance standards in all aspects of case management.
  • Participate in performance improvement initiatives and data tracking.

Qualifications:

  • License: Active New York State RN License (Primary Source Verified)
  • Certification: AHA BLS required
  • Required: Minimum 2 years of case/resource management and utilization management experience
  • Preferred: 3–5 years of care management experience, with working knowledge of EPIC, PRI, and SCREEN tools
  • Skills: Strong assessment, communication, and organizational skills

*Offered pay rate will be based on education, experience, and healthcare credentials.
