Telephonic After-Hours Call - Nights, Weekends, Rotating Holidays

Optum Home & Community Care, part of UnitedHealth Group, is innovating in health care by creating an integrated care model that addresses physical, mental, and social needs, helping patients access care anytime and anywhere.

As a team member of our Senior Community Care (SCC) product, you will provide care to patients at home, including nursing homes and assisted living facilities, supporting improved access to care.

The Home and Community Care (HCC) program offers longitudinal, integrated care delivery. The HCC National On Call APC provides telephonic care and guidance to members and facility staff during evening, overnight, weekend, and holiday hours, managing a designated group of enrollees.

This remote role involves providing after-hours virtual care for aging residents across various settings, offering autonomy and satisfaction in caring for our aging population. It also provides work-life balance and career development opportunities with Optum.

Scheduling

  • This is a full-time, remote position requiring variable shifts on weeknights, weekends, and holidays.
  • Coverage includes:
  • Weeknights from 5 pm to 8 am local time
  • Weekend coverage from Friday 5 pm to Monday 8 am
  • Holiday coverage starting at 5 pm of the last business day to 8 am of the next business day
  • Weekday shifts are 5 PM-2 AM or 11 PM-8 AM
  • Weekend shifts are 11 AM-11 PM or 1 AM-1 PM
  • Holiday scheduling is planned at the year's start.
Experience preference: Recent experience in high-acuity geriatric care (e.g., SNF, LTACH, Hospitalist, ICU, Emergency) is preferred.

Call handling: Expect about 4 calls per hour from nursing staff, RNs, DONs, and others.

Typing skills: Excellent typing skills are essential for live note entry.

If you hold an active New York license, you can work remotely, taking on meaningful challenges.

Primary Responsibilities
  • Handle calls on the telephonic platform, coordinating care between facilities, hospitals, and field staff.
  • Use EMR proficiently to deliver acute care during after-hours shifts.
  • Manage medical and behavioral conditions collaboratively with physicians or specialists.
  • Document encounters accurately to support diagnoses.
  • Complete mandatory training.
  • Monitor member needs and adjust care plans as needed.
  • Review orders and interventions for appropriateness and effectiveness.
  • Communicate effectively with all stakeholders, including physicians and families.
  • Meet shift and attendance requirements.

Required Qualifications
  • Active NY NP license or ability to obtain by start date, with no disciplinary restrictions.
  • Active ANCC or AANP certification in relevant specialties or ability to obtain certification/license in NY.
  • Current DEA license or ability to obtain post-hire.
  • Availability for weekends, weekdays, and holidays.
Preferred Qualifications
  • 2+ years of postgrad experience in Long Term Care.
  • Knowledge of geriatrics and chronic illnesses.
  • Understanding of advanced illness and end-of-life discussions.
  • Proficient in computer documentation and EMRs.
The salary range is $104,500 to $156,000 annually, based on full-time employment, experience, and qualifications. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k.
