Pride Health is hiring a RN Case Management to support our client’s medical facility in Bronx, NY 10461. This is a 8-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization!

Job Summary:

• RN Case Management. 40 hours per week (5x8s, Days: 9 am to 5 pm)

RN Case Management:

• Conducting intake assessments with patients and families.

• Ensuring that families feel sufficiently equipped and supported.

• Developing individualized treatment plans that account for physical, financial, and other constraints.

• Adjusting treatment protocols, as needed.

• Suggesting and coordinating appropriate follow-up services to promote wellness.

• Ensuring that patients have transport to and from their homes and all pertinent healthcare facilities.

Experience required:

• Two years case/resource management and utilization management is required.

• 3-5 years Care Management experience is preferred.

• EPIC experience PRI (Patient Review Instrument) and SCREEN experience/certification ideal

Additional Information:

• Shifts: 36 hours per week (5x8s, Days: 9 am to 5 pm)

• Hourly Pay: $60/hr to $64/hr on W2

*Offered pay rate will be based on education, experience, and healthcare credentials.
