Job summary

LSMA Management in California is seeking a Concurrent Case Management LVN to support inpatient and post-acute care coordination and concurrent review activities to ensure medically appropriate, timely, and cost-effective services for members.

Under RN leadership, the LVN performs concurrent review, monitors utilization, supports discharge planning, facilitates transitions of care, and coordinates with hospitals, SNFs, IRFs, LTACHs, home health, physicians, and care teams.

Qualifications

• Must have CA LVN license in good standing.
• Minimum 2 years LVN clinical experience.
• Experience with concurrent review or MSO/health plan preferred.
• Knowledge of inpatient and post-acute care settings.

Responsibilities

• Perform concurrent review and utilization management.
• Support discharge planning and transitions of care.
• Coordinate services across hospitals, SNFs, IRFs, LTACHs, and home health.
• Maintain accurate clinical documentation and reporting.
• Collaborate with care teams while ensuring HIPAA compliance.
