Job summary

LaSalle Medical Associates IPA seeks an LVN to support concurrent case management and post-acute care coordination within a CA MSO framework.

Under RN leadership, you will perform concurrent review, monitor utilization, assist with discharge planning, and coordinate services across hospitals, SNFs, and home health teams. Emphasis on Medicare Advantage, Medi-Cal, and commercial populations.

Qualifications

• Minimum: High School diploma or equivalent; Graduation from an accredited vocational nursing program.
• Preferred: Training in case management, utilization management, or care coordination.

Responsibilities

• Perform concurrent review and monitor inpatient and post-acute utilization.
• Support discharge planning and transitions of care.
• Coordinate services across the continuum of care with hospitals, SNFs, IRFs, LTACHs, home health, and care teams.
• Ensure compliance with HIPAA and patient confidentiality.

Education
