Job summary

Torrance Memorial is seeking an LVN Case Manager to support physicians and the care team in coordinating utilization management, transitions of care, and discharge planning. This role aims to improve clinical outcomes and patient satisfaction while managing care costs.

The LVN Case Manager will handle a designated caseload, collaborate with interdisciplinary teams, and promote efficient resource use across the care continuum.

Qualifications

• Active RN, LVN, PTA, PT, or OT license required.
• BCLS or ACLS certification required.
• 3+ years of acute case management experience or equivalent (clinical liaison/navigator).

Responsibilities

• Coordinate utilization management, transitions of care, and discharge planning.
• Manage designated patient caseload and optimize length of stay.
• Provide patient/family education throughout care.
• Collaborate with interdisciplinary team to ensure appropriate care settings.
