Qualifications

• Proficient with MS Outlook and MS Office (Word, Excel).

Responsibilities

• Arrange post-discharge services including transportation.
• Issue Important Message from Medicare (IMM) letters.
• Assist with MOON notices and updates to commercial payers.
• Provide discharge planning updates to hospital care team.
• Learn and support Case Management staff under supervision.
