Jobot is seeking a Director of Case Management in Martinez Lake, AZ, to lead an 11-person interdisciplinary team and drive safe discharge planning in a growing hospital expanding to 81 beds by 2027. The role offers an incentive plan and room to obtain CCM/ACM certification within the first year.

Competitive compensation ranges from $100,000 to $125,000 per year, with opportunities to contribute to organizational strategy and collaborate with hospital leadership.

Formación

• Provide leadership for the Case Management department.
• Oversee 11 direct reports (RNs, LPN, MSWs, LSW, and other allied health professionals).
• Experience with hospital case management leadership preferred.

Responsabilidades

• Guide safe, timely, and effective discharge planning across the hospital.
• Support Utilization Review processes in coordination with the case management team.
• Monitor and manage case manager caseloads (typical caseload of 13 patients per CM); step in to carry a patient assignment when census is low or staffing is short.
• Maintain working knowledge of case managers' patient assignments and actively support the team as needed.
• Foster strong interdisciplinary communication across nursing, therapy, and hospital leadership.
• Partner with the RDCM on training, orientation, and ongoing education initiatives.
• Participate in hospital leadership meetings and contribute to department strategy.
