Job summary

Jobot is seeking a Director of Case Management in Yuma, AZ to lead an 11-person interdisciplinary team in a growing hospital expanding to 81 beds by 2027. The role offers a salary of $100,000-$125,000 with an incentive plan eligibility and opportunities to earn CCM/ACM certification within the first year.

Leadership at a high-visibility level includes collaboration with hospital leadership and the Regional Director of Case Management.

Qualifications

• Experience leading a case management team.
• Familiarity with discharge planning processes.
• Knowledge of Utilization Review procedures.

Responsibilities

• Provide leadership for the Case Management department with 11 direct reports (RNs, LPN, MSWs, LSW).
• Guide safe, timely, and effective discharge planning across the hospital.
• Support Utilization Review processes with the case management team.
• Monitor caseloads (typical caseload ~13 patients per CM); cover patient assignments when needed.
• Maintain knowledge of team assignments and support as needed.
• Foster interdisciplinary communication across nursing, therapy, and leadership.
• Partner with RDCM on training and education initiatives.
• Participate in hospital leadership meetings and contribute to department strategy.
