Job summary

Community hospital part of a system is seeking a Director of Case Management to join the leadership team and report to the CEO. The role targets acute care/inpatient experience and will oversee a multidisciplinary CM staff, with a heavy emphasis on workload balance and system-wide coordination.

The selected candidate will be encouraged to obtain CCM/ACM within the first year; the organization covers examination costs and supports relocation and a bonus incentive program.

Qualifications

• CCM or ACM certification required within first year; exam cost covered
• RN or Licensed Social Worker by qualification
• Experience in acute care or inpatient settings
• Experience leading a CM team and coordinating multidisciplinary care

Responsibilities

• Lead the case management department and sit on the leadership team
• Direct reports include 3 RN CMs, 1 LPN, 3 MSW, 1 LSW and allocate workload
• Maintain CM caseload targets around 13 per CM when census permits
• Participate in a bonus incentive program and relocation package as offered by the system

• Director doesn't carry a case load unless census is low or they are short staffed
• Case loads for CM's are around 13
• Looking for candidates with acute care or inpatient experience
