Job summary

Jobot is seeking a dynamic leader to head a Case Management team in an inpatient rehabilitation setting. You’ll drive census management, patient outcomes, and seamless discharge planning, with direct oversight of 11 direct reports and a 58-bed facility.

This fully onsite role reports to the CEO and offers a competitive salary of $105,000 to $115,000 plus quarterly bonus eligibility, plus opportunities to shape interdisciplinary care planning and coordination with payers.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of his/her discipline.
• Active license required if licensure is mandated for the discipline in the state.
• Minimum of three years of hospital-based Case Management experience including Utilization Review and Discharge Planning.

Responsibilities

• Lead the day-to-day operations and HR management of the Case Management department.
• Oversee the interdisciplinary plan of care and discharge planning to ensure effectiveness and appropriate services; focus on census management and patient outcomes.
• Serve as patient/family advocate to meet needs.
• Provide guidance and training to Case Managers on caseload management and policies.
• Review operations to maintain quality.
• Build relationships with payers and healthcare networks.
