• 1–3 years of related experience.

• Provides the CMS forms (MOONS, IMs) to patients in a safe, timely, and effective manner.
• Verifies and assures that documentation in medical records is accurate and confidential.
• Responds to inquiries from CRM leadership and responsible parties.
• Interacts with patients to track progress and ensure satisfaction.
• Adheres to professional standards per protocols and regulations.
• Develops effective working relationships with CRM team.
• Facilitates case coordination and information sharing for DC planning.
• Assists with referrals to PCP and JHS specialty clinics and scheduling.
• Coordinates outpatient studies as needed and demonstrates CARE values.
• May attend management meetings and participate in confidential decisions.
