Qualifications

• RN license required.
• 2 years experience required.
• No travel required.

Responsibilities

• Guide patients and families through orientation, discharge planning, and understanding benefits.
• Coordinate education for patients and caregivers to support treatment planning.
• Develop and implement individualized treatment plans with the care team.
• Complete psychosocial assessments and communicate needs across the team.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values in every patient interaction.
