Job summary

A leading company is seeking a dedicated professional to assist with the transition of care for ambulatory patients. This remote role involves collaborating with care teams, assessing patient needs, and ensuring timely discharges. The ideal candidate will have experience with Epic EHR and excellent communication skills. Flexibility for rotating weekends is required.

Job description

Weekends : on rotation every couple of weeks

Holidays : required

100% remote - Supporting Pacific Time Zone

Must have knowledge and experience with Epic EHR

Must be open to rotating weekends

Description :

Assist with UR transmission, handling a high volume of faxes and voicemails, data entry, and utilizing Epic knowledge.

Coordinate and implement the transition of care (TOC) / discharge (DC) plan for ambulatory patients. Prioritize and coordinate care across the continuum through critical thinking, teamwork, and communication with care providers, patients, families, and external vendors to ensure timely discharge. Responsibilities include:

  • Collaborating with the care team to execute the TOC plan.
  • Assessing patient needs with case managers and social workers.
  • Communicating discharge plans and coordinating post-discharge referrals.
  • Monitoring progress and resolving barriers to timely discharge.

Additional responsibilities include participating in team meetings, documenting accurately in the electronic medical record, providing excellent customer service, and supporting department goals.
