Job summary

MyPath is seeking a dedicated Case Coordinator for its Clinical Services team to align clinical, residential, educational, and external partners in delivering person-centered care for children, adolescents, and adults with special needs.

The role emphasizes treatment planning, communication, discharge planning, and aftercare to ensure continuity of care and positive long-term outcomes within Indiana DCS guidelines.

Qualifications

• Ability to maintain a calm and effective demeanor in emergency and stressful situations.
• Ability to manage health needs in a residential setting.
• Strong clinical judgment and communication skills, both written and verbal.

Responsibilities

• Develop, implement, and monitor Individualized Treatment Plans and Behavioral Support Plans.
• Ensure services are person-centered and compliant with Indiana DCS standards.
• Monitor progress toward treatment goals and revise plans as needed.
• Coordinate admissions, transitions, and discharge planning for students.
• Provide Aftercare services for DCS students post-discharge.
• Meet with assigned students monthly to support engagement and coordination.
• Facilitate meetings such as CST, IEP, transition planning, and court hearings.
