Job summary

ALLCARE HOMECARE LLC is seeking a compassionate Case Coordinator to join our in-person home care team in Long Beach, CA. You will assess clients, develop care plans, and coordinate services with families and providers, visiting client homes regularly.

The ideal candidate has healthcare experience, CPR/First Aid, strong communication and organization skills, and reliable transportation with willingness to travel. This role is not remote and requires on-site visits.

Qualifications

• Healthcare experience preferred.
• CPR/First Aid certification required.

• Ability to work independently and manage multiple client needs.
