Job summary

ALLCARE HOMECARE LLC in Irvine, CA is seeking a Case Coordinator to manage client assessments and develop individualized care plans. This role requires regular in-person visits to client homes and travel to client locations.

You will coordinate home care services with clients, families, and care providers, monitor progress, and update care plans as needed. CPR/First Aid certification is required; strong communication and independence are essential.

Qualifications

• Healthcare experience in home care or case management.
• Experience with individuals with disabilities is a plus.
• Current CPR/First Aid certification.
• Strong communication, organization, and problem-solving skills.
• Reliable transportation and willingness to travel to client homes.
• Ability to work independently and manage multiple client needs.
