Job summary

AllCare HomeCare, LLC. in Irvine, CA is seeking a compassionate Case Coordinator to join our in-home care team. You will conduct client assessments, develop care plans, and coordinate services with clients, families, and providers in person.

Qualifications

• With healthcare experience; home care or case management experience preferred.

• Ability to work independently and manage multiple client needs.
