Job summary

North Metro Community Services, Inc. in Westminster, CO is seeking a case manager for children under three with developmental disabilities or delays. You will complete required paperwork, maintain records, and coordinate services.

Qualifications

Bachelor’s degree in Human Services preferred or 5+ years experience with people with disabilities.
High school diploma or equivalent required.
Combination of education and work experience may meet requirements.

Responsibilities

Complete required paperwork and maintain accurate records in department database.
Communicate professionally and positively with others.
Schedule, attend, and facilitate meetings as required by the department.
Maintain ongoing contact with family and providers, taking action as necessary.
Coordinate, monitor, and change services.
Assist with the transfer of services.
Coordinate referrals and follow-up as needed.
Find and secure funding for services.
Maintain productivity standards as required by agency or government regulations.
