Job summary

Rocky Mountain Human Services is seeking a Case Manager I to perform case management activities for clients in Denver. You will evaluate eligibility for developmental disability services, review documentation, and communicate with families and professionals to support service access and compliance.

The role emphasizes thorough documentation, adherence to state/federal guidelines, confidentiality, and collaboration with the care team to promote independence and quality of life for individuals

Qualifications

• Bachelor’s degree required in human behavioral sciences.
• Experience in case management preferred.
• Strong written and verbal communication.
• Ability to maintain confidentiality and ethics.

Responsibilities

• Review applications, medical records, and evaluations to determine eligibility.
• Evaluate eligibility for Developmental Disability services.
• Communicate with applicants, families, guardians, and professionals to gather information.
• Prepare determination reports documenting findings.
• Maintain records in compliance with regulations and policies.
• Explain eligibility decisions and guidance on appeals.
• Stay current on DD eligibility criteria and best practices.
• Coordinate collaboration among the interdisciplinary health care team.
• Complete forms and documentation accurately and timely.
• Attend staff meetings and participate in training opportunities.
