Job summary

Pyramid Healthcare Martinsburg Inpatient is seeking a Case Coordinator to manage client engagement, coordinate with treatment teams and referral sources, and arrange transportation and aftercare. The role requires strong communication, documentation, and collaboration to support ongoing care within the inpatient program.

The position offers an 8:00 am–4:30 pm schedule, with an hourly rate of $18-$20 depending on education and experience, and requires a High School Diploma, with a Bachelor's

• Bachelor’s degree in a human services related field preferred.

Responsibilities

• Coordinate client engagement through appointments, aftercare plans, schedules and meetings with the treatment team.
• Communicate with referral sources and outside providers; document contacts at admission and during treatment.
• Coordinate transportation needs and provide case coordination sessions.
• Assist in development of aftercare plan; provide referrals and provider contacts.
• Support groups and participate in treatment team collaboration; maintain professional communication.
• Document client, family, referral, and outside provider contacts in Carelogic.
• Collaborate with client navigation for aftercare needs and EOS appointments.
• Complete mandatory training and professional development.
• Other duties as assigned.

Case Coordinator

• Address: Pyramid Healthcare Martinsburg Inpatient
• 64 Boy Scout Road, Hedgesville, WV
• Schedule: Monday through Friday 8:00 am - 4:30 pm
• Hourly Rate: $18-$20/hour (based on education/experience)

Essential Duties And Responsibilities

• Coordination of client engagement through appointments, aftercare plans, schedules and meetings in collaboration with treatment team.
• Coordination and communication with referral sources and outside providers through consistent, professional communication, assessment of needs, and documentation of contact at admission and for the duration of treatment. **This may include communication or coordination for UR, legal, and employment needs for the client.
• Provides case coordination services for clients through coordinating transportation needs.
• Providing individual case coordination sessions. Assists in development of aftercare plan, providing referral and aftercare provider contact, addressing outside provider needs.
• Assists with groups to support clinical services provided, and supporting clients on a weekly basis to support engagement in the program.
• Participates in treatment team collaboration and maintains professional, collaborative communication with counselors and treatment team.
• Documentation of client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines
• Collaboration with client navigation team for client aftercare needs.
• Provides onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and Client Navigation team.
• Participate in mandatory corporate training, program training and professional development.
• Other duties as assigned

Education, Licensure, & Experience

• High School Diploma required
• Bachelor’s degree in a human services related field preferred
