Job summary

Monarch Communities in Weymouth, MA is seeking a Care Manager (Caregiver) to assist residents with daily living per care plans. This non-exempt, hourly role reports to the Health & Wellness Director and offers a supportive team environment.

Responsibilities include bathing, dressing, toileting, feeding, documenting services in ALIS EHR, monitoring changes, and assisting with activities. Requires CNA or CHHA certification and experience with elderly care; flexible schedule and free parking are

Qualifications

• High School Diploma or GED required.
• Certified Nursing Assistant or Certified Home Health Aide certification required.
• Experience with elderly residents preferred, dementia experience is a plus.

Responsibilities

• Provide resident care per care plan (bathing, dressing, toileting, feeding).
• Maintain safety, dignity and confidentiality of residents.
• Document services in ALIS EHR during shifts and report changes to supervisor.
• Assist with meals and activities and support other duties as assigned.
