Job summary

Shadow Mountain Memory Care in Phoenix, AZ is seeking a dedicated Certified Caregiver (Care Partner) to provide high-quality, person-centered care to our residents. Join a compassionate team and help residents with daily living tasks, dignity, and privacy.

Qualifications

• High school diploma or equivalent required.
• AZ Caregiver Certification is required.
• Must be at least 18 years old and able to read, write, and speak English.

Responsibilities

• Provide assistance with activities of daily living following person-centered care.
• Assist with bathing, grooming, dressing, ambulation, meals, and dressing changes as needed.
• Respect residents’ privacy, dignity, and preferences.
• Follow infection control procedures and safety standards.
• Complete required state training upon hire and annually.
• Support daily requirements and tasks as requested.

Certified Caregiver "Care Partner"

Responsibilities of the Care Partner

• Provide assistance with activities of daily living in a manner that supports our person-centered care philosophy.
• Provides personal care services as outlined in the individual services plans such as assistance with bathing, grooming, dressing, ambulation, positioning, assist to meals, feeding, change bed linens, laundry, etc.
• Assist in daily requirements and tasks as requested.
• Ensure residents are provided privacy, respect, and dignity.
• Follows standard precautions and infection control procedures.
• Completes state required training upon hire and annually.

Required Experience

• Possess, at a minimum, a high school diploma or equivalent.
• AZ Caregiver Certification required.
• Be at least 18 years of age.
• Basic computer skills.
• Must be able to speak, read, and write in the English language.

Job Type

• Full Time - PM schedule (2pm–10pm).
• Consistent hours with opportunities for additional shifts when available.
