Job summary

Shadow Mountain Memory Care in Phoenix, AZ is seeking a compassionate Certified Caregiver (Care Partner) to provide high-quality, person-centered care to residents.

The role offers an evening shift (2-10pm) with full-time hours, competitive wages, and opportunities for training and career growth. Candidates must meet AZ Caregiver Certification requirements and be able to speak and write in English. A supportive team environment is provided.

Qualifications

• High school diploma or equivalent.
• AZ Caregiver Certification required.
• Must be at least 18 years old.
• Basic computer skills.
• Must be able to speak, read, and write in English.

Responsibilities

• Provide assistance with activities of daily living per individual services plans.
• Assist with bathing, grooming, dressing, ambulation, repositioning, meals and feeding, and linens.
• Assist in daily requirements as requested.
• Ensure residents' privacy, respect, and dignity.
• Follow standard precautions and infection control procedures.
• Complete state-required training upon hire and annually.

Responsibilities of the Care Partner

• Provide assistance with activities of daily living in a manner that supports our person-centered care philosophy.
• Provides personal care services as outlined in the individual services plans such as assistance with bathing, grooming, dressing, ambulation, positioning, assist to meals, feeding, change bed linens, laundry, etc.
• Assist in daily requirements and tasks as requested.
• Ensure residents are provided privacy, respect, and dignity.
• Follows standard precautions and infection control procedures.
• Completes state required training upon hire and annually.

Required Experience

• Possess, at a minimum, a high school diploma or equivalent.
• AZ Caregiver Certification required.
• Be at least 18 years of age.
• Basic computer skills.
• Must be able to speak, read, and write in the English language.
