What You Will Do

You will help implement personalized care plans that support the whole person — mind, body, and spirit. You will treat clients like family, going above and beyond to enhance their quality of life.

Essential Functions

• Deliver exceptional care in clients’ homes
• Assist with personalized schedules and care plans
• Perform daily tasks safely (ADLs)
• Provide companionship and emotional support
• Assist with personal care, mobility, incontinence, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment
• Transport clients to appointments
• Document tasks and report changes in clients’ conditions
