Job summary

Monarch Communities is seeking a compassionate Care Manager for our Brandywine residence in Haddonfield, NJ. The role includes direct resident care, ADL support, and care plan documentation in a collaborative team environment.

Responsibilities include assisting with daily activities, ensuring safety and dignity, and promoting resident participation in community programs. CNA certification and a high school diploma are required; dementia experience is a plus.

Qualifications

• Certified Nursing Assistant (CNA) or DHS certification required
• High School Diploma or equivalent required
• Experience with elderly population; dementia experience preferred

• Provide resident care including bathing, dressing, toileting, and feeding as per care plan
• Maintain safety, dignity, and confidentiality of residents
• Escort residents to meals and activities
• Document services performed during shift and report changes in condition to supervisor

Location: 132 Warwick Rd, Haddonfield, NJ

Salary Range: $20.50 - $21.00 / Hour

What will you get to do as a Care Manager?

• Use your compassion to provide warm, engaging personal care services, support, and companionship.
• Engage and assist residents with activities of daily living (ADLs).
• Look for changes in residents' day and personal care needs to update their personal Resident Care Plan.
• Encourage resident participation in community programs and activities.\

Key Responsibilities:

• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Escort residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts
• Document all services provided to your assigned residents during your shift
• Report any change of condition in a resident immediately to your supervisor

Qualifications

• Certified Nursing Assistant and/or DHS certification plus High School Diploma
• Knowledge and experience in working with the elderly population, preferred dementia experience

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
