Job summary

Monarch Communities Brandywine at Haddonfield in New Jersey is seeking a Care Manager to provide compassionate personal care, support, and companionship to residents. You will assist with activities of daily living and help update care plans.

The role requires a CNA certification and a high school diploma; prior elder care experience is preferred, with dementia experience a plus. The position offers part-time hours and a supportive team environment with opportunities for growth.

Qualifications

• Must have CNA certification and a High School Diploma.
• Experience caring for elderly residents preferred.
• Dementia care experience preferred.
• Ability to document care in Resident Care Plan.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding per care plan.
• Maintain safety, dignity, confidentiality of residents.
• Escort residents to meals and activities.
• Document all services during your shift.
• Report any change of condition to your supervisor immediately.

Location:132 Warwick Rd, Haddonfield, NJ

Salary Range: $20.50 - $21.00 / Hour

What will you get to do as a Care Manager?

• Use your compassion to provide warm, engaging personal care services, support, and companionship.
• Engage and assist residents with activities of daily living (ADLs).
• Look for changes in residents' day and personal care needs to update their personal Resident Care Plan.
• Encourage resident participation in community programs and activities.\

Key Responsibilities:

• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Escort residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts
• Document all services provided to your assigned residents during your shift
• Report any change of condition in a resident immediately to your supervisor

Qualifications

• Certified Nursing Assistant and/or DHS certification plus High School Diploma
• Knowledge and experience in working with the elderly population, preferred dementia experience

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
