Job summary

Clarity Care, Inc. in Wisconsin is seeking an Activity Support Provider to assist the Day Services Program with daily activities, independence, and community involvement.

You will support members' physical, social, and personal needs and help them achieve personal goals in a compassionate, hands-on environment. Starting pay up to $18/hr based on experience; require a high school diploma, valid WI driver’s license, and background checks.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities

The Activity Support Provider assists with the day‑to‑day operations of the Day Services Program.
