Clarity Care, Inc. in Wisconsin is seeking an Activity Support Provider to assist with the Day Services Program, helping members with daily living activities and personal needs. No experience required; training provided and immediate openings available.

Responsibilities include planning engaging activities, supporting community involvement, preparing meals per dietary needs, and ensuring regulatory compliance while delivering compassionate care to individuals with varying abilities.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.
