Clarity Care, Inc. is seeking an Activity Support Provider to assist with day-to-day operations of the Day Services Program.

You will support members in community and recreational activities, helping with personal needs and goal achievement, while creating engaging activities aligned with member interests. The role requires a high school diploma or equivalent, being 18+, a valid Wisconsin driver's license, and the ability to pass a caregiver background check.

Qualifications

• Must possess a high school diploma/equivalent or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities

• Support members in community and recreational activities.
• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decision-making.
• Create fun and engaging activities tailored to member interests.

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.
