Job summary

Clarity Care is seeking an Activity Support Provider to assist with daily Day Services operations in Oshkosh, WI. You will help members participate in community and recreational activities, support personal cares, and aid in daily living tasks.

No prior experience or certifications are required, and immediate hiring is available. The role involves flexible, hands-on support and a focus on enhancing quality of life for individuals with varying abilities.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver's license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities

• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decision-making.
• Create fun and engaging activities tailored to member interests.

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.
