Hacienda HealthCare seeks Direct Support Professionals to provide person-centered care in group homes, day programs, and employment groups. Responsibilities include daily living support, community integration, safety monitoring, and independence promotion under program leadership.

Roles involve driving residents to appointments, assisting with meals, showers, laundry, and ensuring compliance with ISP objectives. A safety-sensitive position with flexible shifts and on-site work in Phoenix, AZ.

Qualifications

• Must be 25 years or older to drive.
• Must have a clean driving record and be approved by our insurance carrier.
• Must pass Hacienda Drivers Training.
• Must pass Article 9 and Prevention and Support within 90 days of hire.
• CPR/First Aid certification required within 30 days of hire.
• Must obtain and maintain a Class 1 Finger Print Clearance Card.
• MVD driving record to be provided at time of offer.

Responsibilities

• Provide safety, emotional and physical support to residents.
• Assist with daily living activities and community integration.
• Monitor health and safety; document ISP objectives daily.
• Help residents with meals, showers, and laundry; ensure cleanliness.
• Drive residents to appointments and maintain driving records.

Responsibilities include assisting with daily living activities, community integration, skill development, health and safety monitoring, and promoting independence under the direction of program leadership.
