Responsibilities

• Provides support for individuals and/or groups of persons with disabilities in the community.
• Assists the person(s) receiving services in identifying and communicating his or her requests and needs for services and supports.
• Implements services and supports to address the person’s goals and desired outcomes as identified through each person’s Centered Plan.
• Seek to understand the individuals in the context of their personal history, their social and family networks, and their hopes and dreams for the future.
• As a Direct Support Caregiver, help to implement services and supports that will assist clients to achieve personal goals and outcomes as identified through an Individual Support Plan.
• Work with an interdisciplinary healthcare team to identify appropriate support methods necessary to achieve stated goals.
• Help clients identify personal outcomes that align with their interests, cultural backgrounds, and future plans.
• Foster a respectful, supportive relationship with clients, focusing on client-centred care.
• Responsibilities for shifts and daily duties are directed by the Direct Care Manager and vary by assigned shift.

Qualifications

• Education and Experience: Requires a high school diploma or GED with a minimum of two years of experience with the population supported. Additional requirements may include an Associate Degree in a health related area or two years’ experience in a health related field.
• Physical requirements: Regularly lift 30 lbs; able to lift a minimum of 70 lbs; able to pull a minimum of 50 lbs; capable of squatting, kneeling, crawling, crouching, climbing, and stooping; able to stand and walk regularly; able to demonstrate proficiency in CPR in various positions as required; vision and hearing requirements;
• Other: Valid driver’s license and automotive insurance (where driving is required). CPR, first aid, and relevant certifications are a plus. Ability to pass background checks and drug screens.
• Experience with individuals with developmental and intellectual disabilities is preferred; flexible schedule and availability for on-call coverage as needed.
