Job summary

You will support participants in identifying needs, communicating requests, and achieving personal outcomes through their Centered Plan.

The role is entry-level and emphasizes advocacy, teaching life skills, and promoting independence while maintaining confidentiality and safety.

Qualifications

• High School Diploma or GED (Preferred (TN)).
• Conflict resolution skills are required.
• Some computer skills preferred.
• Good communication skills are essential.
• Ability to maintain good work attendance and confidentiality.

Responsibilities

• Promote healthy living and prevent illness and accidents.
• Support individuals in achieving goals and making informed choices.
• Accompany people to work, activities, and community outings.
• Teach independent living skills and provide transportation when needed.
• Document the care you provide and educate on rights.
• Advocate and connect people with community resources.
