West Bay RI in Johnston, RI is seeking a Transition Specialist to help students and young adults (17–21) transition from school to work. You will empower individuals to build life, social, and job skills through personalized coaching, real-world learning, and strong community partnerships.

The role emphasizes hands-on job coaching, resume support, interviews, and finding meaningful employment while promoting independence and successful futures.

Formación

• High school diploma or GED required.
• Experience supporting individuals with disabilities preferred.
• ACRE certification or equivalent preferred.

Responsabilidades

• Personalize career planning through person-centered assessments.
• Build community partnerships with local businesses for work experiences.
• Coordinate job shadowing, workplace experiences, and career exploration.
• Provide on-site coaching and individualized guidance during shifts.
• Develop workplace skills: communication, problem solving, etiquette.
• Foster social connections and teamwork among students and coworkers.
• Assist with resume development, interview prep, and employer outreach.
• Promote independence and goal achievement in career plans.
• Maintain timely documentation to meet school requirements and ORS reporting.
