Job summary

BERKSHIRE FAMILY AND INDIVIDUAL RESOURCES INC. seeks a dedicated Case Manager to oversee group employment referrals and assist individuals served in achieving meaningful employment within the community.

You will perform ISP-based assessments, job evaluations, and create career plans while maintaining compliance with DDS regulations and agency policies. A valid driver’s license and transport to multiple sites are required.

Qualifications

• Associate degree with a minimum of 1 year experience in a supported employment program or High School diploma/GED with 4 years’ experience working with developmentally disabled adults in a supported employment program or in a position of case management.
• Must maintain all certification and background checks as required (CORI, SORI, fingerprinting, LEIE, driving record, etc.).
• Valid US driver’s license and access to a vehicle.

Responsibilities

• Manage the case management of all group employment referrals and members.
• Attend ISP meetings and complete job evaluations and career plans.
• Collaborate with DDS and other funding sources to develop employment opportunities for people served.
