• Valid driver’s license, travel and reliable transportation required.
• Strong Microsoft Office skills; database experience preferred.

Responsibilities

• Assess the situation of clients, identify unmet needs, and arrange for, coordinate and monitor services.
• Advocate with other agencies on behalf of the client.
• Maintain accurate records and documentation.
