Job summary

The State of North Carolina Department of Health and Human Services (DHHS) seeks a Rehabilitation Counselor in the Sylva VR/IL Unit, Waynesville satellite office, to help individuals with disabilities achieve career and independent living goals.

The role includes counseling, developing individualized plans, coordinating services, and outreach to employers to support employment success, with a general/physical caseload and structured trainee progression.

Qualifications

• Knowledge of or experience in case management.
• Knowledge of or experience working with people with disabilities or underserved/marginalized populations.
• Knowledge of or experience working within a team setting.

Responsibilities

• Evaluating professional diagnostic data to determine eligibility and required rehabilitation services.
• Developing a written plan of services with client, parent and/or guardian.
• Providing rehabilitation services within counseling relationships and offering supportive counseling.
• Monitoring client progress throughout the service delivery process.
• Conducting outreach and public relations activities, including employer engagement and job fairs.
• Collaborating with community partners and employers to assist individuals with disabilities in achieving employment success.
• Documenting service delivery and case progress in the case management system as required.
