Qualifications

• Bachelor's degree in human services or related field required or equivalent experience.
• Experience in special education, supported employment, and/or transition service delivery preferred.
• Knowledge of transition practices from school to adult life and person-centered planning.
• Knowledge of benefits planning and work incentive programs or willingness to learn.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Serve as the 'single point of contact' representing the collaborative in the schools regarding all aspects of the project.
• Partner with Vocational Rehabilitation Services (VRS) and support the VRC in identifying eligible applicants and facilitating the VRS process.
• Assist with marketing activities and development of promotional materials for the project.
• Participate in planning transition/career fairs at participating schools.
• Provide support for adaptations and job accommodations for student interns and employers.
• Connect students with resources for benefits analysis and technical support.
• Collaborate with school staff on skill development and student self-determination training.
