About the Role

The Advance Practice Provider, Nurse Practitioner collaborates with physicians, other providers, and families to develop care plans for patients with complex health conditions. Provides clinical and non-clinical support, focusing on urgent care via telehealth for home and community-based patients.

Responsibilities

1. Develop and implement care plans for adult patients with chronic and complex conditions.
2. Gather patient history, conduct physical exams, and develop treatment plans aligned with patient goals.
3. Identify and address gaps in care, and coordinate with interdisciplinary teams.
4. Educate patients and families on medication, illness management, and crisis prevention.
5. Maintain accurate medical records and participate in ongoing education.
6. Prescribe medications within state authority.

Qualifications

• MS in Nursing and active, unrestricted Family or Adult Nurse Practitioner license in Indiana.
• At least 2 years managing complex care cases.
• Experience with Electronic Medical Records (EMR).
• Preferred: DEA registration, experience with developmental disabilities and chronic illness care.

Additional Details

Position is primarily remote, with work hours Monday-Friday, 8:00 am to 5:00 pm CST or EST, including rotating on-call. Reside within 50 miles or 1-hour commute of an Elevance Health location.
