CareBridge Nurse Practitioner: Florida

• Sign-On Bonus: $5,000
• Must have a current, active Nurse Practitioner license in applicable state AND an active RN Compact license.

Location: This position is primarily virtual.

Work Shift: Monday – Friday, 8:00 am to 5:00 pm CST or EST, with rotating on-call.

Responsibilities

• Collaborate with physicians, other providers, and families to develop complex care plans based on patient health status and goals.
• Provide clinical and non-clinical support to patients via telehealth modalities.
• Develop and implement care plans for adults with chronic and complex conditions.
• Gather patient history, conduct physical exams, and develop treatment plans.
• Identify and address gaps in care.
• Educate patients and families on medication use, illness management, and prevention strategies.
• Coordinate care with clinical team members and providers.
• Maintain accurate medical records and documentation.
• Participate in continuing education and maintain licensure and certification.
• Prescribe medications as permitted by state law.

Minimum Requirements

• MS in Nursing.
• Active, national NP certification.
• Valid, unrestricted Family or Adult Nurse Practitioner license in applicable state.
• Valid RN Compact license.
• At least 2 years managing complex care cases.
• Experience with Electronic Medical Records (EMR).

Preferred Skills and Experience

• DEA registration or eligibility.
• Experience with developmental disabilities and chronic illnesses.
