CareBridge Nurse Practitioner: Tennessee

$5,000 Sign On Bonus

• Must have a current, active Nurse Practitioner license in applicable state and an active RN Compact license.

Location: This position is primarily remote.

Work Shift: Monday – Friday, 8:00 am to 5:00 pm CST or EST, with rotating on-call.

The Advance Practice Provider, Nurse Practitioner is responsible for collaborating with physicians, other providers, and families to develop complex care plans, providing clinical and non-clinical support to patients.

Responsibilities include:

1. Providing urgent healthcare via telephone and tele-video to patients in home and community-based services.
2. Developing and implementing care plans for adults with chronic and complex conditions.
3. Gathering patient history, performing physical exams, and developing treatment plans.
4. Identifying and closing gaps in care.
5. Supporting patients and families with education on medication, illness management, and crisis prevention.
6. Coordinating care with clinical team members and other providers.
7. Maintaining accurate patient records and documentation.
8. Participating in continuing education.
9. Prescribing medication as permitted by state law.

Minimum Requirements:

• MS in Nursing.
• Active, national NP certification.
• Valid Family or Adult Nurse Practitioner license in applicable state.
• Active RN Compact license.
• 2+ years managing complex care cases.
• Experience with Electronic Medical Records (EMR).

Preferred Skills:

• DEA registration or eligibility (preferred).
• Experience with developmental disabilities and chronic illnesses (preferred).
