• Position Title: Nurse Practitioner 100% Virtual, CareBridge
• Job Description: CareBridge Health, a member of the Elevance Health family, within Carelon, aims to support individuals in home and community settings to maximize health, independence, and quality of life through home-care and community services.
• Position: Advance Practice Provider - Nurse Practitioner
• *$5,000 Sign-On Bonus
• Qualifications: Active, unrestricted Nurse Practitioner license in Kansas, Arizona, Iowa, or Texas AND an active RN Compact license.
• Location: Primarily virtual role.
• Work Shift: Monday - Friday, 8 am to 5 pm CST, with rotating on-call.

Responsibilities

• Provide urgent healthcare via telephone and tele-video to patients in home and community-based services.
• Develop and implement care plans for adults with chronic and complex conditions.
• Gather patient history, perform physical exams, and develop treatment plans.
• Identify and address gaps in care.
• Support patients and families with physical and psychosocial needs.
• Educate patients on medication, illness, diet, adherence, and crisis prevention.
• Coordinate care with clinical team members and providers.
• Maintain accurate medical records and documentation.
• Participate in continuing education.
• Prescribe medication as permitted by state law.

Minimum Requirements

• MS in Nursing
• Active national NP certification
• Valid, unrestricted Family or Adult NP license in Kansas, Iowa, Arizona, or Texas
• Active RN Compact license
• At least 2 years managing complex care cases
• Experience with Electronic Medical Records (EMR)

Preferred Skills

• DEA registration or eligibility (preferred)
• Experience with developmental disabilities and chronic illness cases (preferred)
