Job summary

Qualifications

• RN licensure in Massachusetts is required.
• 3–5 years of acute care experience preferred.
• ACM/ CCM/ CMAC certifications are preferred.
• BLS certification required.

Responsibilities

• Collaborates with care team to develop plan of care and patient flow.
• Performs initial assessments and identifies barriers to discharge.
• Coordinates transitions and referrals to post-acute providers.
• Communicates plan and discharge date to patient/family and team.
• Participates in multidisciplinary rounds and ensures regulatory compliance.
