Job summary

Baystate Health is seeking an RN Case Manager for a part-time role to coordinate clinical care and outcomes for a designated patient population. You will lead the interdisciplinary team to develop complex discharge plans and ensure efficient transitions to post-acute settings.

Responsibilities include facilitating care plans, conducting assessments, and collaborating with payers and physicians to optimize reimbursement and continuity of care.

Qualifications

• BSN preferred; 3–5 years nursing and hospital case management experience preferred.
• Knowledge of utilization/quality review standards and community resources.
• Strong interpersonal skills for interaction with care teams, payers and patients.

Responsibilities

• Facilitates care plans per plan of care and coordinates discharge planning.
• Leads daily patient care huddles with nursing to ensure timely progress.
• Oversees allocation of hospital resources, patient satisfaction and quality of care.
• Collaborates with payers and physicians for transitions and approvals.
• Performs readmission risk assessments and implements targeted interventions.
