Qualifications

• High School Diploma or GED required or equivalent.
• Two years pharmacy or patient-focused healthcare experience preferred.
• Pharmacy Technician Certification Board certification preferred and NYS registration within 90 days.

Responsibilities

• Perform medication reconciliation at admission with accurate histories.
• Coordinate Meds-to-Beds intake and discharge medication delivery.
• Communicate with pharmacists, nurses, physicians and outpatient pharmacy.
• Document findings in the EMR and support transitions-of-care workflows.
