Job summary

The Transitional Care Clinical Pharmacy Manager at Montefiore Medical Center leads longitudinal pharmacotherapy management for a defined panel of high‑risk patients during hospitalization and after discharge, using telemedicine, RPM, and in‑person care.

The role requires a PharmD, advanced residency or equivalent, NY license, board certification or eligibility, and strong experience in ambulatory or hospital settings; onsite onboarding with potential limited telemedicine later.

Qualifications

• PharmD degree and NY licensure eligibility.
• Advanced training in cardiology/ambulatory care or PGY-2 with board certification and 3+ years of relevant experience.
• Board certification (BCPS/BCACP) preferred or obtained within 6 months.
• Direct patient care experience in ambulatory, hospital, or transitional care settings.

Responsibilities

• Lead longitudinal pharmacotherapy management for high-risk patients during hospitalization and post-discharge.
• Initiate, optimize, and titrate GDMT under CDTM with focus on heart failure, ASCVD, diabetes, obesity, CKD, COPD, asthma, pneumonia.
• Identify high-impact post-discharge opportunities to prevent readmissions and disease progression.
• Solve medication access, affordability, adherence, and related social needs.
• Collaborate with physicians, nurses, and care teams to design high-reliability care pathways.
• Contribute to quality improvement initiatives and scaling of disease-specific care pathways.
